      实验观察抗阻训练对膝骨关节炎(KOA)大鼠软骨组织损伤的保护作用,并探讨其可能的作用机制。实验将30只雄性SD大鼠随机分为三组:正常对照组(正常组)、KOA组(模型组)、运动+模型组(运动组),每组10只。实验采用通过关节腔注射谷氨酸钠碘乙酸溶液法制备大鼠模型,注射第14 d后检测造模成功后进行运动干预。实验结束后,检测关节液TNF-α、IL-6炎症因子浓度和软骨组织TLR4/NF-κB通路相关蛋白表达,在光镜下观察膝骨关节软骨组织的病理变化,并进行Mankin's评分比较,同时计算股四头肌湿重/体重比值。实验结果显示:(1)模型组软骨结构破坏,软骨表面缺损,且有裂隙,各层细胞排列紊乱可见聚簇现象等;运动组上述病理现象明显好转。Mankin's评分结果显示,运动组明显低于模型组(P<0.05)。(2)与模型组相比,运动干预组股四头肌湿重/体重比值显著增加(P<0.01),且高于正常组(P<0.05)。(3)与模型组相比,运动组软骨组织中TLR4、p-NF-κB、p-IκBa的蛋白均明显降低(P<0.05或P<0.01)。(4)与模型组相比,运动组中TNF-α、IL-6浓度亦显著下降(P<0.01)。有规律抗阻训练可增加股四头肌肌肉质量,改善KOA软骨细胞损伤,其机制可能是调节TLR4/NF-κB信号中关键蛋白表达,并减弱其介导的炎症反应。

      To investigate the effects of resistance exercise on cartilage injury in knee osteoarthritis model rats, and explore its possible mechanism. The SD rats were randomly divided into normal control group(normal group),knee osteoarthritis model group(KOA model group), exercise plus model group(exercise group), 10 rats each group. The rat model was prepared by intra-articular injection of sodium glutamate iodoacetic acid solution, 14 days of injection later, the model was successfully established and then exercise intervention was carried out. The levels of TNF-α, IL-6 and TLR4/NF-κB pathway-related protein expression were tested, and the knee cartilage of the rats were treated with HE staining and compared using Mankin’s score among the three groups, and the wet weight/body weight ratios were calculated at the end of the experiments. The results show that the rats in the model group appeared chondrocyte structure necrosis, with cartilage surface defects, accompanied by disorder and arrangement of each layer cells and clustering phenomenon in the local areas. The above-mentioned pathological phenomena in the exercise group improved significantly. The Mankin’s score results showed that the exercise group was significantly lower than the model group(P < 0.05). Compared with the model group, the wet weight/body weight ratios in the exercise group increased significantly(P < 0.01), and were higher than those of the normal group(P < 0.05). Compared with the model group, TLR4, p-NF-κB, p-IκBa protein expressions were significantly decreased in the exercise group(P < 0.05 or P < 0.01). The contents of TNF-α, IL-6 were significantly decreased in the exercise group(P < 0.01). Regular resistance training can increase the muscle mass of quadriceps, and improve the pathological injury of the knee joint cartilage tissue. The mechanism may be related to the weakening of the inflammatory response mediated by the regulation of the expression of key proteins in the TLR4/NF-κB signaling.
