      目的 核心力量训练对妊娠期糖尿病患者血糖控制情况和妊娠结局的改善情况。方法 招募孕期为24~25周诊断妊娠期糖尿病患者64例,通过随机数字列表法将其分为对照组和观察组各32例,对照组实施常规药物治疗和饮食控制,观察组在对照组基础上,进行"普拉提"核心力量训练,并记录其妊娠结局和母婴并发症。结果 对照组相比,观察组第32周、第36周的空腹血糖及糖化血红蛋白测量值比较,差异具有统计学差异(P<0.05)。从胎膜早破发生率和产后出血发生率而言,对照组与观察组间比较无统计学差异(P>0.05),两组间的2型糖尿病、新生儿低血糖、巨大儿发生率的比较具有统计学差异(P<0.05)。结论 核心力量训练有利于提高妊娠期糖尿病患者对血糖的控制;可减少妊娠期糖尿病患者产后向2型糖尿病转变的发生率,并减少新生儿低血糖和巨大儿的发生率;但是对于胎膜早破和产后出血的预防还需做进一步探讨。

      Objective: To improve the blood glucose control and pregnancy outcome of patients with gestational diabetes mellitus by core strength training. Methods: 64 cases of gestational diabetes mellitus patients with gestational period of 24-25 weeks were recruited and randomly divided into the control group and the observation group with 32 cases in each group. The control group received conventional drug treatment and diet control, while the observation group received Pilates core strength training. Results: Compared with the control group, the difference of the fasting blood glucose and glycosylated hemoglobin of the observation group at 32 weeks, 36 weeks was statistically significant (P < 0.05). In terms of the incidence of premature rupture of membranes and postpartum hemorrhage, there was no significant difference between the control group and the observation group
(P > 0.05), but it was statistically significant for the incidence of type 2 diabetes, neonatal hypoglycemia and macrosomia (P < 0.05). Conclusion: Core strength training can improve the control of blood glucose in patients with gestational diabetes mellitus, reduce the incidence of postpartum transition to type 2 diabetes, and reduce the incidence of neonatal hypoglycemia and macrosomia. However, the prevention of premature rupture of membranes and postpartum hemorrhage still need to be further explored.
