      目的 探讨升清降浊方在延缓慢性肾功能衰竭(CRF) 发生发展过程中对重要脏器的保护效用及可能机制。方法 180 只SPF 级 SD 雄性大鼠被随机分为升清降浊方组、升麻单体治疗组、大黄单体治疗组、基本方治疗组、替米沙坦组和空白组,每组 30 只。采用大鼠 5/6 肾切除造模方法制备CRF模型,除空白组不给药物干预外,其余各组连续给药12周。灌药前后采集大鼠血液,检测Scr、BUN、UA等肾功能指标以及C 反应蛋白(CRP)、白细胞介素 1 (IL-1)、白细胞介素 6 (IL-6)肿瘤坏死因子 α (TNF-α) 等炎症因子指标的变化;取大鼠肾脏、心脏和肝脏组织观察不同分组各组织的病理改变。结果 升清降浊方组、升麻单体治疗组、大黄单体治疗组、基本方治疗组、替米沙坦组大鼠Scr、BUN、UA、CRP、IL-1、IL-6、TNF-α 较空白组有明显差异;升清降浊方组、升麻单体治疗组、大黄单体治疗组治疗前后Scr、BUN、UA、CRP、IL-1、IL-6、TNF-α有显著差异;升清降浊方组的Scr、UA、IL-1、IL-6、TNF-α较其它治疗组降低更显著。病理切片结果显示升清降浊方组对大鼠肾脏、心脏及肝脏保护明显优于其他组。结论 升清降浊方可能通过调控炎症因子的表达以达到阻断CRF进展,改善重要脏器功能。

      Objective: To investigate the protective effects and potential mechanism of ascending qing and descending zhuo prescription in the process of delaying the occurrence and development of CRF. Methods: 180 SPF SD male rats were randomly divided into ascending qing and descending zhuo prescription group, rhizoma cimicifugae monotherapy group, rhubarb monotherapy group, basic formula group, telmisartan group and blank group, with 30 rats in each group. 5/6 nephrectomy of rats could be used for establishing CRF model. Except for the blank group, the other groups were given drugs for 12 weeks. Before and after intragastric administration, blood samples of rats were collected to detect the changes of renal function indicators (SCR, bun and UA) and the inflammatory indexes (CRP, IL-1, IL-6 and TNF-α); the renal, heart and liver tissues of rats were observed to analyze pathological changes in different groups. Results: There were significant differences in Scr, BUN, UA, CRP, IL-1, IL-6 and TNF-α of the rats from ascending qing and descending zhuo prescription group, rhizoma cimicifugae monotherapy group, rhubarb monotherapy group, basic formula group and telmisartan group, compared with that of blank group; Scr, BUN, UA, CRP, IL-1, IL-6 and TNF-α of ascending qing and descending zhuo prescription group, rhizoma cimicifugae monotherapy group and rhubarb monotherapy group were notably different before and after treatment; SCR, UA, IL-1, IL-6 and TNF–α in ascending qing and descending zhuo prescription group were significantly lower than those in other groups. The pathological section showed that the protection of kidney, heart and liver in ascending qing and descending zhuo prescription group was better than that in other groups. Conclusion: Ascending qing and descending zhuo prescription could regulate the expression of inflammatory factors to block the progress of CRF and improve vital organ functions.
