| 目的 探讨临床药师参与心内科住院患者的药物治疗并提出药学服务的工作重点。方法 临床药师参与1例高血压病合并心律失常、慢性肾功能不全患者制定治疗用药方案，对降低血压、联合用药、肾功能不全合理用药等进行个体化的药学服务。结果 临床药师及时发现并解决了药物治疗中的相关问题，提出了药学监护要点。结论 临床药师工作的切入点是用药监护以及设计个体化的用药方案。 |

| Objective:To explore the key work for clinical pharmacists to participate in the drug therapy for patients with cardiology and provide pharmaceutical care for inpatients. Methods:Through participating in drug therapy of a patient with hypertension complicated with arrhythmiaand renal insufficiency, clinical pharmacist could propose suggestions for the use of anti-hypertension, Combination Therapy and chronic renal insufficiency and provide individualized pharmacy service for patient. Results:Clinical pharmacist could found and solved the related problems in drug treatment in time and put forward the main points of pharmacology. Conclusion:The entry points of clinical pharmacist participating in clinical medication is administration monitoring and providing individualized pharmacy service for patient. |
