| 目的 探讨和分析射频消融治疗肝巨大血管瘤的临床疗效。方法 回顾性分析我院肝胆外科2010年6月至2015年6月期间应用射频消融治疗69例肝巨大血瘤患者的临床资料。其中50例患者采用超声引导下射频消融治疗，19例患者采用腹腔镜引导下射频消融治疗。结果 56例患者病灶完全损毁，占总数的81.2%，13例患者消融不彻底，进行了第二次射频消融治疗后达到完全损毁。部分患者术后出现发热、肝区治疗部位或射频针穿刺点处疼痛、恶心、呕吐以及不同程度的转氨酶升高，给予对症治疗后好转。没有患者出现腹腔内大出血、胆瘘、空腔脏器穿孔、严重肾功能损害等并发症。术后患者随访时间6~24个月，所有患者的病灶缩小均在50%以上，无1例复发。结论 射频消融治疗肝巨大血管瘤创伤小、安全性高、并发症少、疗效满意，可获得较好的远期疗效，是治疗肝巨大血管瘤的又一新途径。 |

| Objective: To evaluate the clinical effects of radiofrequency ablation in the treatment of giant hepatic hemangioma.Methods: Retrospective analysis of our department of hepatobiliary surgery from June 2010 to June 2015 during application of radiofrequency ablation treatment of 69 cases with giant hepatic aneurysm patients clinical data.50 patients were treated with ultrasound guided radiofrequency ablation,19 patients with laparoscopic guided radiofrequency ablation.Results: The focus of 56 patients were completely damaged,accounting for 81.2% of the total,13 patients with incomplete ablation were treated with the second radiofrequency ablation and then their focus were also completely damaged.Some patients had fever,liver treatment site or RF needle point of puncture pain,nausea,vomiting,and different levels of transaminases,symptomatic treatment was given and then they turn better.No patients had intra abdominal bleeding,biliary fistula,hollow viscera perforation,serious impairment of renal function complications.Postoperative patients were followed up for 6 to 24 months,the focus of all patients lessen above 50% without recurrence.Conclusion: Radiofrequency ablation in the treatment of giant hepatic hemangioma with small trauma,high safety,less complications,satisfactory curative effect,can obtain better long-term effect,it is a new way for the treatment of giant hepatic hemangioma. |
